Sciatica Physical Therapy in New York

One-on-One Sciatica Care with a Doctor of Physical Therapy in Manhattan

Sciatica describes pain that travels along the sciatic nerve, which runs from the lower back through the buttock and down the leg. Symptoms range from shooting or burning pain to tingling, numbness, or weakness, and they typically affect one side of the body. Because sciatica is a symptom pattern rather than a standalone diagnosis, identifying the underlying cause is what guides effective treatment.

At Lifestyle Physical Therapy, sciatica patients are seen by one Doctor of Physical Therapy for the full appointment. That consistency matters when nerve symptoms shift day to day: the therapist who knows your history can adjust manual therapy techniques and exercise intensity in real time rather than working from last week’s notes.

What’s Causing Your Sciatica

Several structural conditions can compress or irritate the sciatic nerve and produce these symptoms. A herniated or bulging disc is one of the most common causes: when the disc’s soft inner material pushes through its outer layer, it can press directly on a nerve root in the lumbar spine. Spinal stenosis, a narrowing of the spinal canal, creates similar nerve compression through a different mechanism.

Degenerative disc disease and osteophytes (bone spurs that form as discs wear down) can also crowd nerve roots over time. In some cases, the piriformis muscle in the buttock tightens or spasms and irritates the sciatic nerve where it passes nearby, producing symptoms that closely mimic lumbar radiculopathy. Pinpointing which of these is driving your pain shapes every decision about how to treat it.

The Lifestyle Physical Therapy Approach to Sciatica Treatment

Appointments run up to 60 minutes, and that time belongs entirely to you and your therapist, with no handoffs to aides or assistants. Treatment for nerve-related pain draws on manual therapy techniques, including spinal mobilization, soft tissue mobilization, and myofascial release, combined with targeted strengthening and flexibility exercises. When your care overlaps with other medical treatment, we coordinate with your physician or surgeon so the plan stays aligned.

Your Sciatica Evaluation & Treatment Plan

The first visit starts with a detailed conversation about your symptoms, medical history, and goals, followed by a movement assessment covering posture, spine mobility, core strength, and functional tasks like bending and lifting. That picture tells us where your sciatic nerve is being stressed and how much load it can currently tolerate.

Early sessions are typically focused on reducing irritation and calming acute symptoms. As tolerance improves, sessions shift toward rebuilding strength and restoring capacity for daily activities. You’ll leave every appointment with written home exercise guidance, and practical tasks like safely carrying bags or climbing stairs are worked into sessions directly so progress made in the clinic carries over outside of it.
