How ACL Rehabilitation Changes Across Each Treatment Path
Your symptoms, surgical status, associated injuries, activity demands, and medical recommendations shape your rehabilitation. Physical therapy doesn’t determine whether you need surgery, but it can address the limitations identified during your evaluation.
Preoperative ACL Rehabilitation
Preoperative rehabilitation may be recommended before a possible ACL reconstruction. Treatment commonly focuses on managing swelling, restoring knee extension and flexion, improving quadriceps activation, and strengthening the hips and lower extremities. Your surgeon remains responsible for recommendations about surgery and its timing.
Nonsurgical ACL Rehabilitation
Some people pursue nonsurgical care based on knee stability, associated injuries, personal preferences, and the demands of work, recreation, or sport. Therapy may address strength, balance, movement control, and tolerance for progressively more demanding tasks. This path should be chosen with guidance from your treating medical team.
ACL Reconstruction Rehabilitation
Postoperative rehabilitation generally begins with protecting the knee, managing swelling, restoring motion, and activating the surrounding muscles. Treatment may then advance to strengthening and single-leg control. When appropriate, later phases may incorporate jumping, landing, running, and activity-specific preparation. Progress should be based on your clinical presentation and rehabilitation criteria rather than the calendar alone. The appropriate medical provider remains responsible for clearing your return to activity.
What ACL Physical Therapy May Address

Depending on your condition and stage of recovery, rehabilitation may address:
